Also work closely with Office on Aging. Also, organize outpatient dialysis and chemotherapy treatments. Conduct family meetings. Document interactions with patients and families, as well as discharge plans. Completed Power or Attorney and Guardianship Paperwork when necessary. Complete treatment plans. Communicate with physician, nurses, physical and occupational therapists regarding discharge plan.
