Summary
Position Summary
The Trauma Social Worker is an integral part of the interdisciplinary team, conducting psychosocial and clinical assessments to develop and implement a comprehensive care plan for traumatically injured patients. The Trauma Social Worker will serve as an educator and advocate for trauma patients and families ensuring emotional and social needs are met throughout the continuum of care. Trauma Social Worker is responsible for assessment and screening of emotional and mental distress following a traumatic event and screens for substance abuse and alcohol use disorders, to provide brief intervention and referrals.
Minimum Requirements
Education
- Bachelor’s in social work
Experience
- One (1) year Social Work experience in a healthcare setting
License/Registration/Certifications
- Licensed as a Social Worker in South Carolina
- BLS Certification
Preferred Requirements
Preferred Education
- Master’s degree in Social Work from an accredited social work program
Preferred Experience
- 3 years of social work experience in a healthcare setting dealing with trauma patients
Preferred License/Registration/Certifications
- LMSW
Core Job Responsibilities
- Utilizes social work best practices including adhering to all applicable federal, state, and local regulations related to providing social work services and respecting a patient's right to self-determination.
- Assess each patient’s unique condition through personal interview, combining data from mechanism of injury, severity of injures, contributing factors to injury, frailty screening, social vulnerability, and healthcare goals to identify support and services needed during medical crisis and throughout the continuum of care.
- Develop and implement comprehensive plans to help patients adjust to sustained injuries/medical diagnosis and transition from the health care system to normal life.
- Provide necessary support for each patient according to health status, personal strengths and weaknesses, emotional state, and additional factors.
- Research local support groups and other resources to determine which ones are right for individual patients to meet their physical, social, financial, and mental health needs.
- Follow up with patients throughout hospital stay, treatment and recovery to determine current state of mental and emotional health.
- Maintain current knowledge of local resources and relevant trauma research to provide best possible recommendation to patients.
- Provides screening, brief counseling, support, and education services to meet the unique mental health challenges of patients and family members as related to trauma, including providing mental health interventions, assisting them in overcoming barriers to social vulnerabilities and recovery.
- Screen traumatic injury patients for risk of depression and PTSD and provides resources as well as referrals to mental health services.
- Screen traumatic injury patients using a substance use and alcohol misuse screening tool and provide brief intervention counseling and referral to treatment.
- Ensure documentation of screenings, counseling, and referral to treatment in medical record for reimbursement coding.
- Work collaboratively with the Case Managers and the treatment team members to develop and coordinate a safe, timely and appropriate discharge plan across the care continuum.
- Work collaboratively with Injury Prevention & Outreach with targeting injury prevention and violence intervention strategies for the community.
- Maintains records/documentation as required.
- Other duties as assigned.
