Job Summary:

The Supplemental Work and Transition (SWAT) RN establishes and maintains communication throughout the process for smooth operations of admitting, discharge, and transfer of patients. The SWAT RN will work independently throughout the hospital to act as a resource to various units during periods of high volume. Additionally, is able to perform general nursing duties in all departments including the regional facilities with adequate supervision.


Job Functions and Duties:

  • <!--[if !supportLists]--> <!--[endif]-->Demonstrates ability to perform a head-to-toe assessment on all patients and reassessments as per policy.  This includes adult, geriatric and the general patient population.
  • <!--[if !supportLists]--> <!--[endif]-->Shows ability to adequately assess and reassess pain.  Utilizes appropriate pain management techniques.  Educates the patient and family regarding pain management.
  • <!--[if !supportLists]--> <!--[endif]-->Demonstrates a time conscious awareness and ability to prioritize
  • <!--[if !supportLists]--> <!--[endif]-->Performs patient care responsibilities considering needs specific to the standard of care for patient's age.
  • <!--[if !supportLists]--> <!--[endif]-->Demonstrates knowledge of medications and their correct administration based on age of the patient and their clinical condition.
  • <!--[if !supportLists]--> <!--[endif]-->Follows the five medication rights and reduces the potential for medication errors.
  • <!--[if !supportLists]--> <!--[endif]-->Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning. Family is included in teaching as appropriate.  Provides emotional support as needed.
  • <!--[if !supportLists]--> <!--[endif]-->Demonstrates an ability to assist physicians with procedures and performs services requiring technical and manual skills.
  • <!--[if !supportLists]--> <!--[endif]-->Demonstrates the ability to start peripheral IV's and use CVAD's.
  • <!--[if !supportLists]--> <!--[endif]-->Demonstrates the ability to care for post-surgical patients.
  • <!--[if !supportLists]--> <!--[endif]-->Demonstrates ability to perform treatments and provide services to level licensure.
  • <!--[if !supportLists]--> <!--[endif]-->Treats patients and their families with respect and dignity.  Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patients and their families.  Functions as liaison between administration, patients, physicians and other healthcare providers.
  • <!--[if !supportLists]--> <!--[endif]-->Interacts professionally with patient/family and involves patient/family in the formation of the plan of care.
  • <!--[if !supportLists]--> <!--[endif]-->Demonstrates knowledge of the principles of growth and development over the life span and the skills necessary to provide age appropriate care to the patient population served.
  • <!--[if !supportLists]--> <!--[endif]-->Able to interpret data about the patient's status in order to identify each patient's age specific needs and provide care needed by the patient group.
  • <!--[if !supportLists]--> <!--[endif]-->Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
  • <!--[if !supportLists]--> <!--[endif]-->Consults other health team members/departments as appropriate to provide for an interdisciplinary approach to the patient's needs.
  • <!--[if !supportLists]--> <!--[endif]-->Communicates appropriately and clearly to Director, Nurse Manager, co-workers and physicians.
  • <!--[if !supportLists]--> <!--[endif]-->Maintains a good working relationship both within the department and with other departments.
  • <!--[if !supportLists]--> <!--[endif]-->Identifies and addresses psychosocial needs of patients and family; communicates with Social Service/Discharge Planner regarding both in hospital and post discharge needs.

  • <!--[if !supportLists]--> <!--[endif]-->Manages and operates equipment safely and correctly.
  • <!--[if !supportLists]--> <!--[endif]-->Coordinates and supervises patient care as necessary.
  • <!--[if !supportLists]--> <!--[endif]-->Demonstrates an ability to be flexible, organized and function under stressful situations.
  • <!--[if !supportLists]--> <!--[endif]-->Maintains a safe, comfortable and therapeutic environment for patients and families in accordance with hospital standards.
  • <!--[if !supportLists]--> <!--[endif]-->Enhances professional growth and development through participation in educational programs, current literature, in-service meetings and workshops; shows responsibility for own professional practice and ongoing education.

Education and Experience:

  • <!--[if !supportLists]--> <!--[endif]-->Graduate of an accredited school of nursing.
  • <!--[if !supportLists]--> <!--[endif]-->Current Registered Nurse License, in the state of Maine, BSN preferred.
  • <!--[if !supportLists]--> <!--[endif]-->Current American Heart Association Healthcare Provider BLS required.
  • <!--[if !supportLists]--> <!--[endif]-->IV Therapy and Phlebotomy experience required.
  • <!--[if !supportLists]--> <!--[endif]-->2 years of nursing experience preferred, 1 year of acute care experience required.
