Position Objective:

The position is responsible for working collaboratively with teams for the purpose of measurably improving patient safety and reducing patient harm/errors. This requires the systematic implementation of effective, reliable care practices and developing an appropriate institutional patient/workplace safety culture.

Essential Job Duties:

  1. Provides input to the organizational patient/workplace safety strategy.
  2. Continuously evaluates and improves the patient/workplace safety program based on internal needs and external requirements and standards.
  3. With appropriate collaboration, executes the development of a comprehensive Patient/Workplace Safety Plan and updates the plan as needed.
  4. Conducts an annual assessment of patient and workplace risks and working closely with the Quality Team, clinical and operational colleagues, develops annual institutional patient/workplace safety goals.
  5. Under the direction of the Director, Quality, develops and implements an institutional patient/workplace safety measurement strategy. Supports/facilitates the implementation of innovation and systems that eliminate patient/workplace harm.
  6. Ensures the development of surveillance systems to identify and mitigate potential patient/workplace harm.
  7. Develops and implements evidence based initiatives to create an appropriate safety culture using transparency as the foundation of this effort.
  8. Working in collaboration with Risk Management, Ambulatory and Hospital Leadership, leads, coordinates root cause analysis of patient/workplace safety events and assures implementation of resulting actions plans.
  9. Assists the Quality Director’s organizational responses to regulatory standards related to patient/workplace safety including but not limited to the JCAHO National Patient Safety Goals.

10. With the collaboration of the Medical Staff, Patient Services, and Human Resources, designs, updates, and implements patient/workplace safety education modules for new employee orientation and continuing education.

Educational/Experience Requirements:

  • RN or other clinical health care field with current license (can be MA, LPN, etc.)
  • Minimum of five years of clinical experience
  • Basic understanding of care processes provided in multiple areas of the hospital and ambulatory settings.
  • Prior HRO implementation responsibility with a proven track record of implementing change in a complex environment.
  • Knowledge of rapid cycle improvement theory and human factor principles
  • Specific patient/workplace safety training
  • Bachelor’s Degree required, Master’s Degree preferred

Working Conditions, Equipment, Physical Demands:

There is a reasonable expectation that employees in this position will be exposed to blood-borne pathogens.

Physical Demands – Light Work

Light work. Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects. If the use of arm and/or leg controls requires exertion of forces greater than that for sedentary work and the worker sits most of the time, the job is rated for light work.
