Summary

Position Summary

 

Assist in the day-to-day operations of the Structural Heart program. Help manage the patient’s care to increase the efficiency and quality of care that is delivered to the patient. This role will start with the time of the patient’s diagnosis and will continue through the patient’s journey throughout their care. Will be the patient advocate and care for them in a highly supportive and personal manner and also serve as a liaison to ensure the patient’s and family’s receive resources and services as needed.

 

Minimum Requirements

 

Education  

  • Graduate of an accredited school of nursing.

 

License/Registration/Certifications

  • Current R.N. licensure in the state of South Carolina
  • BLS, ACLS, PALS/ENPC

 

Experience

  • Minimum of two years of RN experience required.

 

Preferred Requirements

 

Preferred Education  

  • BSN

 

Preferred Experience 

  • Preferred experience includes a background in one or more of the following: Cath lab, cardiac, telemetry, critical care, operating room, interventional radiology, ER, med/surg.
  • Broad Knowledge of Cardiac disease processes, Cardiac Imaging, and/or cardiac community healthcare

 

Core Job Responsibilities

  • Provide patient education pre/peri/Post procedure regarding diagnosis.
  • Contacts every newly diagnosed patient at the time of diagnosis/referral.
  • Ensures and coordinates new patient referrals, scheduling of exams, procedures and appointments, identifies potential gaps as well as the coordination of care for patients returning to their communities (outside of MGC network) for treatment and follow-Uup.
  • Frailty/KCCQ12 for procedure patient at baseline, 30 day and one year.
  • Assist in data collection
  • Establish follow-up for patient’s post procedure with structural heart clinic and cardiology
  • Request records from outside providers for patient’s that have followed outside of SMC/MGC network
  • Data entry into tracking system for continued patient follow-up (Excel spreadsheet)
  • Call patient post procedure day 2–3 for readmission reduction.
  • Liaison on for in-patient nursing units and cath lab staff.
  • Liaison on for patients and families between departments
  • Attendant heart team meetings
  • Schedule and coordinate biweekly multidisciplinary conference
  • Assist in resolving insurance denials
  • Upkeep of patient medical records, tracking forms in summary
  • Assist in filling out FMLA, insurance forms and other pertinent documents for patient
  • Work with team regarding creation of patient education materials
  • Other duties as assigned
