Summary
SIGN ON BONUS $10,000.00 - Eligible Experienced RN
We are hiring a Nurse Admit & Discharge to join our Home Health team!
- Full Time - 8:30am to 5:00pm
Under general supervision, the Registered Nurse demonstrates thorough knowledge of therapeutic patient care, patient advocacy, clinical pathways/care maps, team dynamics, hospital systems, and serves as a resource for the registered nurse and support staff. Collaborates with leadership in developing and implementing the Spartanburg Medical Center culture, Vision, Mission and goals.
It is the premise of the Professional Practice Program that each of the key responsibilities builds on the preceding requirements.
Education
- Graduate of an accredited school of nursing.
License/Registration/Certifications
- Current RN licensure in the State of South Carolina or Compact State
- BLS
Additional Performance Expectations Specific to Unit:
- Completes all admission (OASIS) documentation, including history, medication reconciliation, and vaccination status
- Identifies skilled need, primary diagnosis, and reason for Home Health services
- Orders correct visit frequency based off patient diagnosis
- Gathers information from patient, caregivers, Primary Care Physician, and electronic medical record for OASIS submission
- Schedules follow-up appointments as appropriate including face-to-face, if needed, for admission
- Collaborates with Quality to ensure all required documentation has been captured, closed, and synced.
- Completes all discharge documentation including OASIS assessment and discharge education
- Discontinues all goals and interventions at discharge
