Summary

Position Summary

The Sleep Navigator, under the guidance of the Department Manager and/or System Director, serves as a liaison between Sleep Services and the clinical care team to promote seamless transition from patient referral to diagnosis and compliance.  Participates in the assessment and patient throughput in all services provided, facilitate patient education session, perform chart review and participate in the accreditation process and all applicable quality improvement projects.  The Sleep Navigator will communicate directly with a multidisciplinary team including all referring physicians.

 

Minimum Requirements

Education

  • Associate degree

Experience

  • 3+ years clinical experience in Sleep Medicine
  • Polysomnography scoring competence and reporting

License/Registration/Certifications

  • BRPT’s Registered Polysomnographic Technologist (RPSGT) credential and/or NBRC’s Sleep Disorders Specialist (SDS) credential.
  • BRPT’s Certified in Clinical Sleep Health (CCSH)
  • AASM Adult Scoring Certification
  • Must have and maintain current Basic Life Support (BLS) certification

 

Preferred Requirements

Preferred Education

  • Bachelor's degree in science or healthcare administration

Preferred Experience

  • 5+ years clinical experience in Sleep Medicine
  • 1-2 years of durable medical equipment (DME) experience including compliance data review
  • Supervisory or management experience

License/Registration/Certifications

  • BRPT’s Pediatric Certification
  • AASM Pediatric Scoring Certification

 

Core Job Responsibilities

  • Chart review of orders placed by non-Board-Certified Sleep Specialist in accordance with American Academy of Sleep Medicine accreditation guidelines.
  • Meet chart review competencies required in an open referral model.
  • Create ancillary orders in EPIC for non SRHS physician referrals.
  • Retrieve missing documentation and diagnostic testing reports to facilitate sleep testing authorization and the ordering of durable medical equipment (DME) for patients diagnosed with sleep apnea.
  • Collaborates with the Lead Technologist and referring Physician offices regarding seamless transition from in-lab outpatient diagnostic testing to follow up and next steps.
  • Work with Marketing as needed to develop patient instructions and education materials including but not limited to information on the website
  • Develop and monitor My Chart screening questionnaires and After Visit Summaries.
  • Track patient progress through diagnostic and treatment processes and follow-up with patients to ensure recommended services are received.
  • Maintain relationships with Durable Medical Equipment companies acting as a liaison between the ordering physician and the patient.
  • Ensure efficiencies, resulting in strong clinical documentation required for ordering of therapeutic modalities.
  • Develop and facilitate patient education sessions and educate patients on long-term compliance to maintain optimal therapy, including mask desensitization.
  • As an accredited lab by the American Academy of Sleep Medicine (AASM), the Sleep Navigator will ensure the In-Lab diagnostic testing adheres to all policy and quality assessment for accreditation renewals.
  • Proactively collaborate with providers, community resources, and other colleagues to help patients achieve the best possible outcomes.
  • Assists the Sleep Services Clinical Manager with marketing, outreach, provider education and growth of the in-lab outpatient Sleep Services program.
  • Actively participates in all aspects of accreditation processes established by American Academy of Sleep Medicine
  • Consistently demonstrates excellent customer service, SRHS Standards of Behavior, and service recovery skills to enhance the patient experience.
  • Supports and demonstrates commitment to the mission, vision, standards of behavior and goals of SRHS.

Quality / Performance Improvement / Patient Safety

  • Develops Quality Assurance (QA) measures and maintains supporting data
  • Develops and maintains policies that align with accreditation standards.
  • Annually reviews QA measures, goals and policies.
  • Stays abreast of advancement in technology and patient testing devices, making recommendations to Manager and Sleep Services Board of Directors.

Other responsibilities

  • Capable of highly independent work as well as being a team player
  • Ability to work closely with a diverse group of individuals, from patients to physicians, and industry representatives.
  • Serves as a resource and actively provides direction and supervision to other staff caring for patients in the Sleep Center
  • Mentors and develops Lead Technologists.
  • Function as a Staff Technologist with patient assignments.
  • Provides healthcare instruction/education to patients and family members.
  • Performs delegated duties and other duties as assigned by the Management
