Overview
This role serves as a transitionalist that allows the problematic transition points of admission and discharge to be streamlined through a coordinator. Benefits are noted, such as decreased length of stays, increased face time with providers, increased transition care focus, and patient education. Goals would also be to enhance throughput efficiency. Creating a space for more direct provider care and less indirect care activities are paramount. Increasing provider satisfaction is also noted in facilities that have published their experiences. Much of the research is anecdotal at present, and the role is flexible based on facility need.
This person acts as a liaison from patient to provider to case management for needs post hospital to be better provided for. They can provide list management for the providers to increase geographical rounding adherence and allow providers to spend more face time as they begin their day and allow nocturnists to handle the case load with decreased distractions.

Qualifications
Essential:
- RN-Registered Nurse licensed to practice in Delaware
- BLS-Basic Life Support
- Acute care hospital experience
Competencies Skills

- Clear Communication Skills: Written And Verbal
- Able To Keep Confidential Information Regarding Patients, Team Members
- Able To Withstand Crisis Situations
- Customer Service and Service Recovery Skills where applicable
- Knowledge And Experience with Electronic Health Records
