SUMMARY OF RESPONSIBILITY - Job Summary (general statement of purpose or objective of position)
The Discharge Hospitality RN is accountable for improving patient flow and discharge efficacy; ensuring patients receive care in the appropriate setting; enhancing the quality of the patient experience in a safe, comfortable environment. He/she functions as an advanced practitioner in the role of clinician, educator, care coordinator, and advocate. Facilitates patient flow (throughput) by accessing services for discharged patients; direct supervison of DHC Patient Care Techs; managing, coordinating and monitoring resources to achieve optimal level of independence and health for patients and families; achieving clinical outcomes; monitoring utilization; service, and financial goals by performing the roles of assessor, planner, facilitator and advocate. The Discharge Hospitality nurse is expected lo demonstrate clinical competence and critical thinking.
MINIMUM REQUIREMENTS - Knowledge, skills, abilities, license, registration, certification, education, and experience
RN with current licensure; Healthcare CPR, Communicates well with others; 3 years recent experience in a clinical setting with a minimum of 1 year of supervisory experience preferred -charge nurse or team leader. Knowledge and education, including but not limited to the following; clinical expertise, consultation skills,teaching/learning principles, interpersonal skills, research process, change theory, management skills, and financial analysis. Case Management or adult acute care (Med-Surg, Telemetry, or Critical Care) experience preferred. BSN preferred.
