The Director of Population Health Case Management is accountable for providing vision and strategy for of the Care Management and Population Health programs to impact population health outcomes while meeting all State, Federal and Accreditation requirements and activities designed to achieve quality and value based care objectives in the University of Maryland Population Health Services Organization (PHSO). The Director will lead and manage clinical program operations including program deployment, enhancements, case management team performance, strategic planning, and is responsible for patient care coordination and utilization medical management for the PHSO.
The Director provides clinical leadership for all case care management programs by serving as a role model and change agent to enhance the care delivery model for all PHSO payer relationships. The Director directs daily team operations and provides strong leadership through training, coaching, teaching, and managing team members and programs. In addition provides supervision of case manager supervisors who assist with the management of the team.
What You Need to Be Successful:
Education and Experience
- Bachelor’s Degree in Nursing (BSN), Health Care Administration, or business-related discipline; or equivalent combination of education certifications and work experience is required.
- A minimum of ten years of management experience in a managed health care setting required.
- A minimum of five years of nursing experience in population health, case management or acute care preferred.
- Current, active Maryland Registered Nurse (RN) licensure required.
- Case management certification preferred.
