Shift: Varies
Working Hours: Varies based on home visiting schedule. Typical hours will be 8am-5pm; some weekends.
Summary:
Community health workers (CHWs) play a vital and unique role in linking diverse and underserved populations to health and social service systems. Community Health Workers will primarily be working out in the community with specific target populations. CHW’s will work closely with medical providers, primary care teams, and other agencies to improve patient care and outcomes. Community health workers (CHWs) have a close understanding of the community they serve. This trusting relationship enables them to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. Community health workers also build individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy.
Other information:
EDUCATION: College degree in health related field (preferred); LANGUAGES: English; Spanish (orally and written) preferred for some communities.
CERTIFICATIONS: Must successfully complete the Community Health Worker (CHW) Certification within 6 months of hire date. Valid Driver's license.
COMPETENCIES: Knowledge about community resources appropriate to the need of patients/ families; Effective time management practices (able to organize time, prioritize projects efficiently and effectively) de-escalation skills; problem solving skills and managing change and conflict; and knowledge of computer software. Able to develop trust and effective work relationships with staff members, individuals, families, and external and internal professionals; strong information gathering/ interviewing techniques.
WORK EXPERIENCE: Two years community service experience preferred; Medical terminology and one year work experience in medical office setting preferred; Excellent interpersonal skills with special emphasis on oral and written communication skills. Knowledge of Electronic Medical Record (EMR) preferred.
PHYSICAL REQUIREMENTS: May need to move around intermittently during the day, including sitting, standing, stooping, bending, and ambulating. May need to remain still for extended periods, including sitting and standing. Work evenings and weekends may be required. May need to lift boxes or carry objects. Possess dexterity to operate a PC and other office equipment.
PERSONAL PROTECTIVE EQUIPMENT: Follow standard precautions using personal protective equipment as required.
This job will be authorized 80.00 hours bi-weekly.
