Position Objective:

The Care Management (CM) Clinical Specialist is responsible for coordinating and implementing CM evidenced-based practice (EBP) and services to deliver high quality, cost-effective health care, and outstanding service to patients through expert clinical practice, research, consultation, leadership, and education. The clinical specialist works with system leaders, teams, staff and practitioners to design, implement, and evaluate CM EBP within the Luminis Health system, and facilitates program development, staff orientation and development, and project management.

Essential Job Duties:

  • Expert Clinical Practice – Determines gaps in knowledge, knows the literature pertaining to particular (focused) populations, provides expert input into CM clinical practice guideline development, and identifies meaningful quality indicators and metrics.
  • Research – Performs literature searches and identifies quality sources of data, evaluates evidence summaries for strength and applicability, understands statistical analyses, procures published clinical practice guideline, and evaluates outcomes for EBP projects.
  • Consultation – Collaborates with clinical teams to proactively design and implement population and facility specific clinical practice guidelines and services.  Leads process improvement projects for the care management team.
  • Leadership – Serves as a change agent for evidenced-based decisions and ensures a culture and infrastructure conducive to supporting evidenced-based practice.
  • Education – Informs administrators and clinicians regarding the benefits and resources needed for EBP, disseminates the results of EBP changes, educates Care Management Staff and facilitates staff orientation and development.

Educational/Experience Requirements:

Required Minimum Education:  Master of Science in Nursing (MSN) Degree.

Required Minimum Experience:  Five years in a health care delivery system, and previous applicable experience in case management (outpatient desired), population health management, and/or value based initiatives.

Required Licensure/Certification:  Registered Nurse license in Maryland.

Essential Qualifications:

  • Ability to work with multiple system sites and stakeholders both owned and independent.
  • Ability to understand payer contracts and incentives.
  • Ability to build strong partnerships across the continuum of care
  • Knowledge of transition planning principles, case management and utilization review principles.
  • Advanced knowledge of psychosocial factors and how these relate to wellness is essential.
  • Effective leadership skills.
