The position of Nurse Case Manager at Thomas Jefferson University Hospital is a challenging and yet exciting position.  The Case Manager is the leader of care coordination and the development of the discharge plan. Communication and collaboration with a multi-disciplinary team that includes the medical providers, social work, nursing, ancillary services, patient/family, post-acute providers and payor are all integral to the successful delivery of efficient, quality care and safe, comprehensive discharge planning.


Primary Responsibilities  



  • The Nurse Case Manager actively manages the coordination of care and the discharge planning process through collaboration with the multi-disciplinary team.  The Case Manager will assess the post-hospital needs of patients, families and significant others and facilitate plans appropriate to the patient needs. Evaluates identified clinical, psychosocial and age specific needs of patients to determine priorities to coordinate care as the patient moves through the system during hospitalization. Assesses, oversees and coordinates discharge planning needs for assigned patients. Assessment and evaluation of the patient needs will begin at the time of admission and will be re-evaluated regularly during the hospitalization.  This includes monitoring the discharge plan implementation while identifying and addressing the patient’s psychosocial and support-systems issues.
  • Collaborates with the physicians/providers to ensure the appropriate admission status, and performs clinical reviews utilizing Interqual guidelines for selected groups of patients. Facilitates the patient’s plan of care with an emphasis on the discharge plan, resulting in a decrease in the fragmentation of hospital services and supporting quality care delivery while reducing length of stay.
  • The Nurse Case Manager advocates for the patient and family throughout the entire episode of care, is available to the patient/family as a resource to facilitate communication among providers and remains involved until the planned level of functional health is achieved by the patient and/or discharge criteria are met.  Interviews patients, families and/or interested parties to assess needs and develop an individualized discharge plan. Maintains communication with patient/family as the patient progresses to update/revise the discharge plan
  • Good communication skills and the ability to work and interact with co-workers, visitors and other staff /team members is essential.


Bachelors Nursing Required

Certification for Case Management preferred.


Registered Nurse Pennsylvania


2-3 years as an acute care staff nurse.

Case management and Interqual experience required.
