The Social Worker is a member of a dynamic, fast-paced social work team supporting the behavioral health and substance abuse needs of Beth Israel Deaconess HealthCare (BIDHC), a large network of primary care practices. The team responds to referrals of patients with unmet behavioral health needs through telephonic outreach. The Social Worker will complete assessments and match patients with appropriate behavioral health resources. The Social Worker will contribute to the development and delivery of behavioral health focused training sessions to primary care staff and providers and may additionally provide in-person triage of referred patients within the primary care practice setting.
Job Description:
Screen and triage referred patient requests for assistance around behavioral health, substance abuse, domestic violence, psycho-social stressors and/or concerns around aging at home. Upon receipt of referral, conduct thorough assessment of patient needs and provide information regarding appropriate community based resources. Communicate with patients telephonically and provide short-term intervention with the aim of connecting patients to community-based resources. Utilize motivational interviewing and problem solve around barriers to accessing necessary care. Partner with the multidisciplinary team to facilitate continuity of care planning. Assess patient needs during high-risk moments including concerns of suicidality, psychosis, elder abuse and domestic violence. Work collaboratively with the care team to create a plan to ensure patient safety. Provides timely and appropriate communication and documentation in the patient medical record. Work collaboratively with social worker team to maintain and update database of community-based resources. Maintain up to date knowledge and utilize available behavioral health resources, including partial programs and substance abuse treatment options. Serves as a content resource for multidisciplinary staff and mentor/trainer to less experienced staff. Encourages an environment of idea sharing and feedback. Provides consultation and education regarding psycho-social and behavioral health issues affecting patient care. Advises and addresses ethical and risk management issues in a timely manner. Seek opportunities and implements goals for own professional development. Participates in case reviews and regular team meetings. Serves as a mentor and/or training resource to less experienced staff. Supports an environment of idea sharing and feedback. Collaborate with Community Health Workers, to address concrete resource needs (ie food, housing, transportation) and ensures safety for high risk patients. Build relationships with primary care practices. May be embedded into the primary care practice(s), to allow for in-person assessments and a smooth transition of the patient from the PCP. Social Workers will ensure that PHI (Patient Health Information) will only be used for the purposes of patient TPO (treatment, payment and operations).�� BIDHC patient personal health information is only shared with individuals who are actively managing the patient's care.� NO other parties are entitled to receive, discuss a patient's personal health information without the patient's permission. The Social Worker will ensure patient safety practices to prevent/eliminate risk of patient harm. Adverse patient events and near misses will be promptly reported to the BIDHCRisk Management Dept.
