As an RN Coordinator, you will support our Pediatric Multidisciplinary Programs of Craniofacial and Infectious Disease at the Children’s Health Center. You will function as an advanced practice nurse coordinating all clinical activities for pediatric patients with complex diseases in the outpatient setting. Responsibilities include, but are not limited to:
- Deliver comprehensive case management, including nursing services, patient education, coordination of medical authorizations, referrals, and follow-up care
- Evaluate and streamline patients’ care regimens, developing and implementing diagnostic and therapeutic treatment plans
- Oversee discharge planning and ensure smooth outpatient care coordination
- Primary point of contact for cases requiring follow-up investigations with the LA County Department of Public Health
- Serve as the Nurse Coordinator for assigned California Children’s Services (CCS) Team Center Care clinics
Salary Range: $59.46-88.39 Hourly
