Job Summary: The Clinical Pharmacy Specialist serves as a leader in clinical pharmacy practice in solid organ transplant. This position is the Clinical Pharmacy Specialist in solid organ transplant and will rotate between both inpatient care teams and ambulatory clinic direct patient care. Specialists provide comprehensive clinical and distributive medication management services and will utilize their advanced clinical skills in the provision of evidenced-based, safe, and cost-effective drug therapy to achieve optimal medication therapy outcomes. They serve as a drug information and educational resource for the pharmacy staff, patients, residents/students, medical staff, nursing staff, and other healthcare professionals. The Clinical Pharmacy Specialist will participate in the development of clinical guidelines/protocols, new programs/services, and facilitate professional development activities in their area of expertise.

 

 

 

Job Description:

Essential Responsibilities:

  1. Practices pharmacy in accordance with laws, rules and regulation of the Massachusetts Board of Registration in Pharmacy, Department of Public Health, U.S. Food and Drug Administration, and Drug Enforcement Administration and in accordance with established departmental policies and procedures. Maintains credentials in accordance with the Massachusetts Board of Registration in Pharmacy.
  2. Functions as a therapeutic expert and performs pharmacy consultations per policy guidelines. Works collaboratively with providers to provide guidance on appropriate use of medications, clinical pharmacology, pharmacokinetics, disease state management, and nutrition. Recommends evidence-based medication therapy regimens and monitoring plans.
  3. Works collaboratively with the clinical pharmacy coordinators to develop and implement evidence-based medication guidelines/standard operating procedures/policies. Attends and actively participates in hospital committees pertinent to specific practice area.
  4. Enters/verifies medication orders in the computer processing system accurately and efficiently. Ensures appropriateness of all medication doses in reference to patient's diagnosis, weight/body surface area, age, allergies, and drug interactions.Clarifies incomplete and unclear orders. Suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed per formulary guidelines.
  5. Precepts PGY1 and PGY2 pharmacy residents, pharmacy students, and contributes to staff development in area of specialty practice. Works collaboratively with the clinical pharmacy coordinator to develop the pharmacy skills of pharmacy staff members by teaching and exemplifying those skills to the team members.
  6. Provides educational information/seminars to physicians and other Licensed Independent Practitioners (staff), nursing staff, and other allied health care professionals as needed or assigned. Facilitates collection and analysis of medication-use data per predefined criteria and formulates appropriate recommendations for implementation.
  7. Provides patient consultations, assessments, and educations as needed to meet patient care needs. Documents patient care activities appropriately in a manner specified by the pharmacy department. Oversees and directs pharmacy support personnel including the daily activities of technicians.
  8. Serves in a professional capacity on unit-based decentralized teams. Acts as a liaison to the nursing units, in assigned practice area, in order to communicate pertinent information and to facilitate communication. Accurately re-establishes priorities based on workload and patient needs and ensures that medication doses are delivered within established departmental guidelines.

Required Qualifications:

  1. Bachelor's degree in Pharmacy required. Doctoral degree in Pharmacy (PharmD) preferred.
  2. License Massachusetts Pharmacist required.
  3. 1-3 years related work experience required.
  4. 1 year Pharmacy residency or 3 years of Clinical Pharmacy experience in hospital setting.
  5. Experience with computer systems required, including web based applications and some Microsoft Office applications which may include Outlook, Word, Excel, PowerPoint or Access.

Preferred Qualifications:

  1. Pharmacy Residency PGY2 specialty experience or equivalent experience or 5 years of clinical pharmacy experience in a hospital setting.
  2. Board certification in a Pharmacotherapy specialty.
  3. Project management and program development experience a plus. Pharmacy residency preceptor experience preferred. Coaching and mentoring of pharmacy staff experience preferred. Demonstrates advanced pharmacy knowledge related to area of practice.

  3. Independence of Action: Ability to set goals and determines how to accomplish defined results with some guidelines. Manager/Director provides broad guidance and overall direction.
  4. Written Communications: Ability to summarize and communicate in English moderately complex information in varied written formats to internal and external customers.
  5. Oral Communications: Ability to comprehend and communicate complex verbal information in English to medical center staff, patients, families and external customers.
  6. Knowledge: Ability to demonstrate in-depth knowledge of concepts, practices and policies with the ability to use them in complex varied situations.
  7. Team Work: Ability to act as a team leader for small projects or work groups, creating a collaborative and respectful team environment and improving workflows. Results may impact the operations of one or more departments.
  8. Customer Service: Ability to provide a high level of customer service and staff training to meet customer service standards and expectations for the assigned unit(s). Resolves service issues in the assigned unit(s) in a timely and respectful manner.

Age based Competencies:
Employees in this job must be competent to provide patient care to the following age groups: Neonatal:Birth to 6 months, Youth: 6 months to 16 years, Young adult: 16-30 years, Middle Age: 30 - 60 years, Elderly: 60 -.
Physical Nature of the Job:
Light work: Exerting up to 20 pounds of force frequently to move objects. Some elements of the job are sedentary, but the employee will be required to stand for periods of time or move through out the hospital campus
